Elijah Heart Center (EHC), is a healthcare organization focused on cardiac health. The facility is equipped to handle the full spectrum of cardiovascular services for physicians and patients. The hospital also provides outpatient services for less invasive procedures and clinical care. Although the organization’s patient volume is stable and increasing in volume rapidly, there is a deficit in regards to profitability. As the senior financial consultant, I will present plans for short term and long term goals if needed. I will also recommend specific measures to modernize the hospital and provide specific plans for hospital expansion. Financial Portfolio Elijah Heart Center has managed to stay in operation due to excellent patient services. In the process of great patient care, poor financial decisions have been made in the past that now hinder the profitable spectrum of the organization. Research data revealed that issues that have affected this organization’s financial budget. The data revealed that (EHC) gave large discounts to manage care companies. The nursing staff was affected because higher wages were paid to outside agencies who supplied contract nurses. Of course when dealing with government health funded insurance carriers such as Medicare, the reimbursement levels are well below budget standards. Insurance rates are not current and based on past medical costs which stunts the financial growth of the hospital. Liabilities have increased and ? of the liabilities are related to accounts payable. The hospital equipment will need replacement soon due to extensive usage. Another issue is the constant placement of unused equipment in patient’s rooms. This causes conflict because if the equipment is placed in the patient’s room, it is considered is supposed to be charged to the patient. Phase 1 Capital Shortage Bridging a working capital shortage is one of the strategies that can help increase the hospital’s revenue if a true concrete plan can be formulated. Once all data was received, suggestion from the Executive Board was taken into consideration before any final decision was to be made. The main focus to be considered while bringing forth a strategic plan, is to understand the healthcare business as a whole. According to Baker and Baker (2009), The health care industry is a service industry. It may have inventories of medical supplies and drugs, but those inventories are necessary to service delivery, not manufacturing functions. With this information in mind, two specific cost cutting options were chosen geared toward staffing and patient care. The first option addressed was to decrease the staff hired from outside sources. Nursing and other employees who were hired via contracts worked for higher rates of pay. This rate is normally double the amount of the staff employee. Depending on the specific position and pay grade, large quantities of contract workers drains the current financial budget and reserves. The goal being strived for is the ability to take money being paid out to contract workers, and use it to hire staff at a reasonable rage of hire. This leads into the second option that was chosen. Changing the skill mix is a great strategy to help retain employees, add to their skills to make them more of an asset, and increase the employee morale. It is known that without contract staff to supplement nursing the strain of patient care would increase. That is why it is necessary to utilize the staff already in house that known the routine to be open to learning more skills. The asset to this strategy is that the nurses who are hired for full time status will enter an organization that promotes advanced clinical learning. The projected outcome of this plan is a net savings of at least 90% the first year, and an increase of financial savings by the second net year. Loan Options A decision in regards to loan options is a strategic method that can be harmful to the company deficit if not chosen correctly. After consulting with the executive team, the decision to select a refurbished loan with a lower interest rate of 9% was better than selecting a new loan with an interest rate of 9. 5%. Having the option to refinance a new loan would not be as lucrative or flexible in the first years of loan repayment. Outcome of Decision The outcome of these two decisions showed major improvements among the internal/external working environment as well as decreased overloaded expenses. The loan (option 2), was the best choice @ $1,500. 000, with a low interest rate of 9. 00%. The interest rate is lower than loan (option 1), at 9. 45% interest. The Monthly payments of $131. 177 versus $131. 490 was also appealing. The cost cutting strategies worked for (EHC) and improvement was immediately seen. Phase 2 Funding Options for Equipment Acquisitions The working capital shortfall is now under control at (EHC). With the increased patient flow, the technological aspects of the hospital must now be addressed. After meeting with the Board of Directors, Gilbert Sanchez stated the desire to purchase medical equipment to continue to provide excellent care to clients. The option was given after consultation to either buy new or refurbished medical equipment by acquiring a loan, or acquiring the equipment on lease (capital or operating). In large healthcare organizations, there is constant competition between departments for funding request for new equipment and supplies. According to Baker and Baker (2009), the reason for new equipment is needed must be clearly stated. The acquisition cost must be a reasonable figure that contains all appropriate specifications. The number of years useful life that can be reasonably extended from the equipment is also an important assumption. Mr. Sanchez provided all the necessary information needed. A different and daring approach was used to purchase the equipment needed for the hospital. The High Speed CT Scanner, X-Ray Machine and Ultrasound were all purchased on a Refurbished Equipment Loan. The optimal choice was to purchase the High Speed CT Scanner on a Refurbished Equipment Loan, the X-Ray Machine on a Capital Lease. The choice made for this issue was concrete. The most cost efficient method was used to revamp the equipment in use at the present time. The refurbished loan amount was purchased at a 9 % interest rate. When checking the balance sheet, the total assets and total liabilities were the same at $230. 621. Phase 3 Options for Capital Expansion Now that the capital shortage and equipment acquisition were addressed and the financial improvement of the hospital is rising, there is now a need for added space. The executive committee have plans to add 100 new private rooms as well as consider the expansion other departments such as surgical suites, endoscopy, surgical suites, and women’s service. Other expansions include 5 operating suites along with seven Cardiac Catheterization Labs. Also, twenty critical care patient rooms were also on the list to be added. The options available for selection included, Tax-Exempt Revenue Bonds, HUD 343 Loan Insurance Program and Private Bank Funding. I chose Private Bank Funding. The interest rate is slightly higher than the other options but the Net Present Value (NPV) was better than the total cost of the project. The total cost was $75,000 and the (NPV) came to $180. 250. According to Baker and Baker (2009), the Net Present Value, is a discounted cash flow method. It is based on cash flows in that it takes all the cash (incoming and outgoing) into account over the life of the equipment over this life of the equipment ( or if applicable, over the life of the relevant project). The strategic collaboration between the Board Executives and myself resulted in a great outcome, bringing overall improvement to the organization. I learned the importance of financial budgeting and streamlining with the focus on staff and patient satisfaction. I honestly would not change my decision on this simulation. I feel confident in my decisions as the consultant. I will take what I have learned from this assignment and apply the methods used to maintain a competent financial budget as well as monitor and maintain adequate employee staffing ratios.

There is a point here. Bad ethics isn't necessarily new, but there does seem to be more problems that are even bigger today than yesterday. There is a fundamental reality that seems to have escaped our notice: Ethical issues are important, and ethical locations are not all that uncommon. Much of this does not make it to the nightly news, but ethical transgressions are quite common in today's society.Think about your work life. Are you involved in discussions about strategically important problems, opportunities and questions? Ethics do not serially involve the big transgression all of the time. They can be the result of hidden forces that many times we don't even see. They are so common that we often take them for granted, almost.How often do we take action and don't even think that there is an ethical point to consider? After the summer of 2002, it should be pretty clear that it makes no sen se to knowingly break the law. Nowadays asthma affects one in five households in the United Kingdom, and i t is increasingly common. So how do you recognize the condition and minimize the impact on you, your child and your family? â€Å"(Barlow 2007). Often times parents of children with asthma are not given much information on what to do with their child after discharge from the Emergency department especially in patients who are newly diagnosed with asthma.This results in them bringing back their child even though the asthma attack is mild due to parents/carers knowledge about asthma or on home care is limited. Recent developments in the management of chronic illnesses such as asthma offer many possibilities for optimal control, but parents/main carers of children with asthma need to take on responsibilities for self-care. This paper reflects an informative approach on paediatric asthma. The aim is to give information about asthma, to give a brief definition, explain the symptoms of asthma, what the treatments are, what exacerbates the symptoms, and what to do if an asthma attack occu rs.The better the parent / carer and the child understand asthma and its treatment the better they will be able to manage and control it. It will also set out why a patient information leaflet should be introduced to help patients and carers to better manage symptoms at home and help reduce hospital attendances. Overview Whilst working in a busy north London Accident and Emergency ( A&E ) department It has been noted that when children attend with asthma they are discharged from the Department with no further information or support with regards to home care, especially the ones who are newly diagnosed.What happens often is that the parents are given the medications by the nurse without even explaining what to do next or what to watch out for with regards to their childs asthma because the nurse has limited time due to the fact that the department is often busy and there is no time to sit down and chat with the parents prior to discharge. After collaborating with the peadiatric secti on of the department a need has been identified for giving information to parents / carers about asthma in children to help reduce / prevent A&E attendances and help children be cared for at home by means of their parents and general practitioners(GP).Through writing an action plan I have identified what areas of information was needed to be covered in the information leaflet, these being: What is asthma? At work I have encountered parents who have limited knowledge about asthma.It is a key issue for parents/ carers to know what asthma is In the leaflet, I have briefly mentioned the definition of asthma as derived by the World Heatlh Organization. T he World Health Organization defines asthma as a condition that affects the airways, it is due to the inflammation of the air passages in the lungs and affects the sensitivity of the nerve endings in the airways so they become easily irritated. in an attack, the lining of the passages swell causing the airways to narrow and reducing the flow of air in and out of the lungs. (World Health Organization 2006)The leaflet also contains what signs and symptoms to look out for prior to an asthma attack. Basing from my own experience growing up with my youngest brother who suffered from asthma, I can remember my parents panic as soon my brother developed cough with mild shortness of breath which often times becomes a trip to the nearest hospital. What are the signs and symptoms? The information leaflet includes what symptoms to look out for when an asthma attack occurs. Out of all the research and reading that I have done, I have come up with four common symptoms.The common symptoms of asthm a, they are cough and wheeze which are constant or intermittent in nature. The child may also become breathless, and develop a feeling of chest tightness. the severity of symptoms differ from child to child – from mild to severe. The leaflet serves as a guide in what to do when these symptoms occur. What can make asthma symptoms worse? A part of the information leaflet is about what makes the symptoms of asthma worse. What factors can cause the exacerbation of asthma or â€Å" trigger factors â€Å" .According to the American Lung Foundation, â€Å" the cause of asthma is not fully understood, but it is thought that it may be a combination of genetic and environmental factors, that asthma attacks are often triggered by certain conditions or stimuli. † ( American Lung Association 2007 ). The common triggers of asthma are exercise, infections, allergy, irritants, weather and emotions. â€Å"Exercise induced asthma (EIA) affects about 40-90% of children with asthma, an d occurs when a child's asthma is triggered by exercise or physical activities. Symptoms f coughing, wheezing, chest pain and difficulty breathing usually begin a few minutes after starting the activity and worsen over the next 5-10 minutes. Symptoms usually continue for about 20-30 minutes. â€Å" ( keepkidshealthy. com 2008 ) â€Å"Severe episodes of asthma are often triggered by respiratory tract infections including flu. Research shows that these infections are most frequently caused by virus more than bacteria. Bronchodilator medication, sufficient hydration and steroids (if indicated) are needed to control an asthma attack caused by viral infections.Allergies can trigger an asthma attack in children with asthma. † ( American Lung Association 2007 ). Allergies can trigger an asthma attack in children with asthma. â€Å"Histamine is released during an allergic reaction causing mucous to be excessively produced, the lining of the airway becomes swollen then muscle contra ction in the airway thus leading to an asthma attack. Allergens associated with asthma are common things like dust mites, feathers, moulds, pets, insects, pollens and ingested food such as egg, soy milk, etc.These causative agents can cause minimal reactions sometimes which are of no obvious consequence but daily exposure to the allergens can cause in gradual worsening of asthma. † (American Lung Association 2007 ). How can someone actually say that all religious authorities are concerned about the dangers of nuclear war or all politicians are concerned about nuclear war? Has anyone polled all religious authorities or politicians? There can be a very few religious authorities that might think it will never happen, so there is no reason to be concerned about it, or there could also possibly be a small religious faction that believes that nuclear war will get rid of all the evil in the world and is Gods way of cleansing the world. The same can be said for politicians. Maybe there are a few that would want to start a war because of their selfish motives. One can argue that there are politicians in Syria and Iran that would love to bomb the U.S and Israel. Nothing was said about the politician having to be from this country. The second reason is that it is an illogical conclusion. One can not say that just because politicians and religious authorities share the same concerns that they will become one in the same. They just share one interest. It takes more than caring about nuclear war to become a religious authority. Religious authorities must go through a process that usually takes years of religious education, and practice. There are certain instances, of course, where some politicians are religious authorities also; however that is not predicated on the fact that they share the same view on one certain issue. â€Å"If the Social Security system is further weakened, the elderly will have to fear poverty. Therefore, if the Social Security system is not further weakened, the elderly will not have to fear poverty. This is an  overgeneralization and an illogical conclusion. It is true that if the Social Security system is further weakened, the some of the elderly will have to fear poverty, but not all of the elderly. Some elderly people are very wealthy. Other elderly people have family members who have made arrangements to take care of them and their financial needs. Of course, there will be a lot of elderly that will fear poverty without the social security checks they would be receiving once a month. Because of these reasons the argument is an overgeneralization. The argument is also an illogical conclusion because some people regardless of their social security will still be in poverty. Some people do not have a significant work history to receive a lot of money from the social security department. According to the social security department website, the average check for a retired worker is $1,294 a month. (Office, 2014) According to the United States Government the poverty level for a family of one is $980.83. (Federal Register, 2015) What must be taken into consideration is that some recipients will be well below the average social security check, and some will be well above. Those that are well below the average will be in poverty if that is all they are receiving. This argument does not account for the elderly that will not even receive a social security. They will have to fear poverty regardless of how weak or strong the social security is. â€Å"The anti-abortionists say that the fetus is human, but they have not proved it. Therefore, they have no reasonable basis for opposing abortion. This argument is invalid because of at least two reasons. The first reason it is not a valid argument is because it has been proven that the fetus is human after eight months. How could it not be human if it comes from two persons who have intercourse and conceive a fetus? This is the definition of a fetus according to Merriam-Webster an unborn or unhatched vertebrate especially after attaining the basic structural plan of its kind; specifically: a developing human from usually two months after conception to birth. In this argument, there is no mention as to how soon after conception the fetus becomes a human. Could the argument still be valid if it were eight months after conception? Another reason this is an illegitimate argument is that the argument wants a person to think that the  only reason antiabortionist oppose abortions is because the fetus is human. There are antiabortionists who have many reasons why they feel abortion is wrong. Some of the most popular reasons why some people oppose abortion are because of morality. They believe that the fetus is a human the minute it is conceived. Some people believe it to be divine intervention from God and humans do not have the right to go against Gods desires and plan. Many of these people are very religious in faith and to them it does not matter when doctors say the baby goes from being an egg to a human fetus. Another reason people oppose abortions is because of the safety of the person having the abortion. Some people feel it is an unnecessary risk to a womans health.
